Privacy notice for online scheduling

This notice explains what happens to the information you give us when you book online. It sits alongside the practice's Notice of Privacy Practices, which you can ask for at the office.

What we collect

Your name, date of birth, home address, email, mobile phone, the type of visit and the time you choose. We don't ask why you're coming in, and we don't collect insurance or payment details online.

Why we collect it

To schedule your visit, match you to your chart, and send you a confirmation, reminders and any changes. We don't sell it, share it for advertising, or use tracking cookies.

How it's protected

• Everything is sent over an encrypted connection (HTTPS).
• Your details are encrypted before they're stored, with a key only the scheduling system holds.
• Only Dr. Joshi's office can open them, after signing in with a one-time code. Every time someone opens your details, it's recorded.
• Emails to you include your first name and appointment time, never your date of birth or address. Emails to the office include no patient details at all.

Your choices

You can change or cancel online with the link in your confirmation email. To correct your details or ask what we hold, call the office at 732-910-1134. If you'd rather not book online, just call.
